Required Forms for Fall and Winter 2010


Please complete the following required forms:


FOCUS Medical Form

The FOCUS Medical Form and Consent Form is required for every student. It will only need to be completed once per year. If you updated the medical information this past Summer or Fall, you will not need to submit another medical form unless any medical information has changed.

In order for us to be prepared in case of injury or emergency, every student must have an updated and accurate FOCUS Medical Form and Consent Form on file in order to participate in any program activities.

If your child has any food allergies, please be very specific so that our kitchen staff can be fully prepared.

Fall  & Winter 2010 Travel Form

The FOCUS Travel Form needs to be completed in order that FOCUS will be able to prepare for coordinating the travel of your child. If you have questions about traveling, please contact your local staff about the best way to travel from your area. 

Ski Waiver and Rental Forms 

There are additional ski waiver and rental forms for Winter House Parties. After registration, these forms will be emailed out.
