Required Forms for Summer 2010
Please complete the following two required forms. Please note, if you are attending Island Cruise, you have a separate packet that you will find below.
FOCUS Medical Form and Consent Form
The FOCUS Medical Form and Consent Form is required for every student. It will only need to be completed once per year. If you updated the medical information this past Fall or Winter, you will not need to submit another medical form unless any medical information has changed.
In order for us to be prepared in case of injury or emergency, every student must have an updated and accurate FOCUS Medical Form and Consent Form on file in order to participate in any program activities.
If your child has any food allergies, please be very specific so that our kitchen staff can be fully prepared.
FOCUS Travel Form
Please do not make any travel plans until you recieve the official Registration Confirmation via email from the Programs Office. The FOCUS Travel Form needs to be completed in order that FOCUS will be able to prepare for the pick up and departure of your child. If you have questions about traveling to the Vineyard, please view our Travel Information or contact your local staff about the best way to travel from your area.
For students who are registered for Island Cruise, please read through the Information Packet and complete the included forms.
