Giving By Mail

Thank you for your interest in giving to FOCUS. When giving by mail, please download, print, and fill out this form and mail it to the address below (along with your check if that is how you choose to make payment).  On this form you will be able to choose whether you would like to make a one-time or monthly gift, and can choose to give by check or credit card. 

Once you have filled out the appropriate form, please send it to this address.

Please be sure to indicate how you would like your gift designated.  If you are giving to a specific area, staff member in an area, or to a scholarship fund, the annual fund, or otherwise please indicate as such in the memo line of your check, or as part of an included instruction. To see a complete list of our funds, go to the "donate online" page and view the drop down menus as a reference.  Simply viewing this page does not mean you must give online.
